Does leucocytosis identify bacterial infections in febrile neonates presenting to the emergency department?

چکیده
OBJECTIVE This study was undertaken to evaluate the discriminatory power of the peripheral white blood cell (WBC) count to identify bacterial infections in a cohort of febrile neonates (or=38 degrees C, underwent lumbar puncture, and had a WBC count obtained were included. They were divided according to microbiological and radiographic findings into four groups: bacterial infections, viral infections, pneumonia, and negative sepsis evaluations. RESULTS A total of 69 febrile neonates met the inclusion criteria. The number of neonates in each group was as follows: 8 with bacterial infections, 10 with viral infections, 3 with pneumonias, and 48 with negative sepsis evaluations. There was substantial overlap in WBC counts among the groups. The area under the ROC curve was 0.7231 (95% CI 0.5665 to 0.8797). CONCLUSION In a cohort of febrile neonates evaluated in the emergency department, the WBC count had modest discriminatory power in identifying neonates with bacterial infections and demonstrated substantial overlap among groups. The present data suggest against the use of any WBC count threshold to identify bacterial infections in febrile neonates presenting to the emergency department.

Does serum procalcitonin have a diagnostic value in febrile adult patients presenting to the emergency department?
Hausfater and colleagues stated that in febrile adult patients presenting to the emergency department (ED) a procalcitonin (PCT) ≥0.2 mcg/l can help physicians to identify bacterial/parasitic infections [1]. We disagree and want to illustrate that by calculating likelihood ratios (LR). A LR is a semi-quantitative measure of the performance of a diagnostic test, expressing the magnitude by which...
متن کاملAssessment of Febrile Neonates According to Low Risk Criteria for Serious Bacterial Infection
OBJECTIVE It is propounded that febrile neonates with low risk criteria (LRC) can be carefully observed without parenteral antimicrobial therapy; but yet, reliability of LRC to exclude serious bacterial infection (SBI) is uncertain. METHODS The records of all febrile term neonates, seen in the emergency room and admitted in neonatal ward of 17 Shahrivar children's hospital of Rasht, Iran from...
متن کاملUnpredictability of serious bacterial illness in febrile infants from birth to 1 month of age.
BACKGROUND A prior study (N Engl J Med. 1993; 329: 1437-1441) produced an effective screen to identify 1-to 2-month-old febrile infants (FIs) who are at low risk of having a serious bacterial illness (SBI). Because of anticipated differences in the epidemiological features of febrile illnesses, that Philadelphia protocol was not applied to FIs younger than 1 month. OBJECTIVES To describe the ...
متن کاملSerum procalcitonin measurement as diagnostic and prognostic marker in febrile adult patients presenting to the emergency department
INTRODUCTION Identification of bacterial infections is crucial if treatment is to be initiated early and antibiotics used rationally. The primary objective of this study was to test the efficiency of procalcitonin (PCT) in identifying bacterial/parasitic episodes among febrile adult patients presenting to an emergency department. Secondary objectives were to identify clinical or biological vari...
متن کاملShould We Evaluate Febrile Young Infants Step-by-Step in the Emergency Department?
In this issue of Pediatrics, Gomez et al 1 present a new “Step-by-Step” algorithm for management of febrile young infants. Although bacteremia and bacterial meningitis, termed invasive bacterial infection (IBI), 2 occur in only 1% to 2% of febrile young infants, 3, 4 clinicians fear “missing” these infections due to potential for mortality and morbidity.5, 6 It is longstanding practice that you...
